Missing on piece of puzzle and, maybe the final key.

Grandfather looks like the guy that played in the GodFather - and his name is on the tip of my tongue. driving me nuts! Can't see the tip of my tongue.

Called friend in PA - now feeling really dumb. That was him and that was the name of the character in the movie .. which shows I didn't watch the movie. So don't know what he called his gun. HELP or didn't watch enough gangster movies. also had dinner while away.
Marlon Brando was the actor I was trying to think of.
Got it .. a lucky guess.

Where do you use the lighter? It is not on the cigar in the ashtray. There are no candles in the room. Where?

Use the lighter on the stain on the desk in second view right of the cigars. It gives you four numbers written in Italian. user google translate.
