I can move the cigarette but I've got no clue what do do with it and I can also only pick up 2 of the keys from the big pile but neither of them work.

oh yeah, and how do you get out?

in room one when you have opened drawer and removed all the money there is a tiny white thing that you can move put it on the red thing in ashtray it is now dynomite put it on door handle then light it with cigarette it will blow up then find the right key

use the razor on little green scratch on head board then click it again this takes some time to find right spot it will move the picture and the key is behind it to open door
