Note: all found using the slider on the table
1. Table
2. Wall
3. Bottom right corner, right side of door
---after lighting all the lights on the table---
4. Behind moveable panel on wall
5. Where the yellow box used to be

Hmm it is hard to explain what is happening there. It took me 20 min to do it the one think I can say is

there are 3 buttons you can start with. by using those 3 buttons as headers you can figure out their combination

one of those 3 buttons is at the right of the dots the second or 3rd dot in the middle row if I remember correctlyu
