do you mean the thing in the box in the night drawer?!
you must open that with a sort of pin

For the gameboy/PDA you have to take your note with the "pass" on it, replace the roman numerals with decimal numbers (I explained it some posts ago how) and solve the mathematical puzzle. This puzzle changes in every game (well not in every game, but there are several possibilities)

Yes, that was one of the bad endings (I think there are 6-8 bad endings and only 1 good ending). You should not leave through the door and only leave in the night.
