Rarrr. I actually gone really really far. But I'm stuck for now.
Anyway,

Find a rock at the screen left of the gates. Combine the fishing line with the rock and use it on the mechanism at the far right.

I think that the point is to make everybody happy...
I have one key left, 12 coins, a knife, a medallion, a sandwich, a balloon and a pearmain (?).
Been to all places and stuck.

I have

balloon, knife, 1 key and pearmain. I'm missing 1 egg. I gave the really tall guy one medalion (from fish) but he didn't gave me anything...

To the left of the butcher's market is a place called aqueduct. There's a wall on the right which has a face on it. click it and stuff its mouth with the fruit.
